Coalition, Primaris (Missouri’s Quality
Improvement Organization), and Dartmouth Medical
School have been working under a grant sponsored
by the Commonwealth Fund to help educate physicians
and the community about the Ideal Missouri Practice
(IMP).
The Ideal Missouri Practice (IMP) is a patient-centered,
efficient, accountable, and accessible practice
style with low overhead. The program is based
on extensive testing and work by faculty at Dartmouth
Medical School and is the core component of IMP.
More than 30 practices nationwide have achieved
significantly better results on quality measures
such as access, continuity, efficiency, and patient-centered

How’s Your Health? (HYH) is IMP’s
patient assessment tool designed to help physicians
and patients cross the quality chasm by communicating
more fully. Completed by consumers, it identifies
potential health problems, the patient’s
level of functioning, clinical symptoms, and patient
satisfaction with care. HYH also provides the
individual participant with their own results
with online access to educational information
specific to those results ---- immediate feedback.
